I am adopting the dog solely as a domestic pet for myself and/or my family and will not use the animal for any illegal activities, including fighting. 2. I agree not to physically abuse, neglect, starve, or harm the pet in any way. 3. I agree to spay or neuter the dog if it has not already been done, and I will ensure this is completed within the time frame specified in the spay and neuter voucher provided. 4. I grant the Rescue visitation rights to confirm compliance with this agreement. 5. I confirm that neither I nor anyone residing in my home has ever faced legal action for animal cruelty or neglect. 6. If I am a tenant, I confirm that my landlord has consented to this adoption and I authorize the Rescue to contact my landlord for verification. 7. I agree to comply with all state and local animal control ordinances in my community. 8. I will ensure that the dog is examined by a licensed veterinarian at least once a year and receives appropriate vaccinations and treatments for good health. 9. I will provide sufficient food and water to maintain the dog's healthy weight. 10. I will ensure that the dog has adequate shelter. 11. I will provide the dog with the necessary daily exercise. 12. I will not allow the dog outside without proper identification. 13. I will comply with all laws regarding leashes and waste removal. 14. I will train the dog to prevent it from harming others. 15. If the adoption does not work out, I agree to return the pet to the rescue before seeking any other placement. 16.
